SE Health is actively seeking both full-time and part-time Registered Practical Nurses to serve areas including Minden, Lutterworth, and Blairhampton. Registered Practical Nurses will conduct client assessments, create care plans, and facilitate care transitions while collaborating with community services. This position emphasizes ongoing support and professional development with a team-driven approach to community healthcare.
Key Responsibilities:
• Conduct comprehensive client assessments and develop care plans
• Lead primary care for designated client caseloads
• Collaborate with community support services for care transitions
Requirements:
• Current registration with the College of Nurses of Ontario
• CNA specialty certification is an asset
• Valid driver’s license and travel required
• Current CPR certificate and updated immunization records
• Willingness to complete required certifications
