Key Responsibilities:

• Provide scheduled community visits to assess patient needs

• Deliver safe and ethical nursing care in diverse environments

• Implement and adjust care plans based on ongoing evaluations

• Establish rapport with patients through collaborative practices

• Document and maintain accurate patient records during encounters

Requirements:

• Valid CNO registration as a Registered Practical Nurse

• Possession of a valid driver’s license and vehicle

• Experience in community care is an advantage

• Adaptability and reliability for scheduled visits

• Willingness for on-call duties if required
