Key Responsibilities:

• Develop and evaluate comprehensive care plans

• Assess patient goals and discharge planning needs

• Act as a liaison for health community resources

• Manage patient transitions ensuring accountability

• Participate in peer reviews and team meetings

Requirements:

• Associate's of Nursing degree required

• RN license is essential

• Current BLS certification required

• Preferred 1+ years in an acute care setting

• Strong ability to comply with safety standards and clinical protocols
