Continuing Care Manager in Two Hills

Location: Two Hills

Lead the Central Zone Rural Team as a Continuing Care Manager for Two Hills Continuing Care Centre. Drive quality healthcare while managing an interdisciplinary team focused on excellent client care.

This role requires strong leadership to guide a team of healthcare professionals at the Two Hills Health Centre. As the Continuing Care Manager, you will collaborate with Area Leaders and various stakeholders to ensure quality care in alignment with the policies of Alberta Health Services. Your focus will be on promoting team development and effective change management while coordinating necessary staffing and clinical tasks.

Key Responsibilities:

• Provide leadership to an interdisciplinary healthcare team

• Collaborate with Area Leaders and care providers

• Ensure ethical and competent client care delivery

• Manage staffing and clinical tasks effectively

• Liaise with stakeholders to integrate care services

Requirements:

• Bachelor’s degree in nursing (or health related)

• Current registration with CRNA

• At least five years’ nursing experience

• Two years’ management experience in a unionized setting

• Knowledge of CCHSS and accreditation standards
