As a Community Support Worker, you will engage in case management and intake processes, ensuring that individuals receive the care and resources they need. The role requires collaboration with families and service providers, making it essential to possess strong interpersonal and organizational skills. Previous experience in human services and knowledge of social work principles is crucial.

Key Responsibilities:

• Conduct intake assessments for clients seeking support

• Coordinate with families for service access and application

• Represent clients' needs in multidisciplinary team settings

• Uphold CTRC policies and social work ethics

• Provide assistance in navigating government services

Requirements:

• Diploma in Social Services or related field

• At least two years of relevant experience

• Understanding of social work codes and ethics

• Valid G2 driver’s license and access to a vehicle

• Bilingual proficiency in English and French is beneficial
