Location: Timiskaming North (Iroquois Falls A)
Enhance community well-being as a Registered Social Service Worker at Northern College. Join a supportive team focused on helping patients and their families access essential resources.
You will report to the Executive Director, East End Family Health Team, playing a crucial role in facilitating pathways to community services. In this role, coordinate patient care, advocate for clients, and educate families about diagnosis and treatment. Your skills will directly impact the lives of individuals and their loved ones while fostering collaboration with community partners.
Key Responsibilities:
• Facilitate referrals to community services and supports
• Advocate for patients with social service agencies
• Coordinate care schedules and arrange transportation
• Liaise between patients and their medical team
• Develop new support programs with team input
Requirements:
• Two-year diploma in Social Service Worker or related field
• One year of public interaction experience required
• Current registration with the relevant social workers' college
• Clear criminal record check for vulnerable sector
Contribute to enhancing well-being while building valuable community connections at Northern College.
