Position: Social Worker for Disabled Young Adults
Location: Peterborough County and North Hastings County (Lakefield)

Join us at the forefront of disability services as a Senior Social Worker focused on the 0-25 Adult Disability Team. Based in either Stevenage or Hemel Hempstead, this role offers a flexible hybrid work model.

You will be responsible for guiding young adults with autism and physical disabilities through critical transitions into adulthood. This role requires a strong foundation in social care laws, particularly EHCPs and the Care Act, as you manage complex cases and work collaboratively across sectors. Your leadership will ensure that young adults receive personalized care that promotes independence.

Key Responsibilities:

• Lead case work for young adults with disabilities

• Ensure effective transitions from education systems

• Collaborate with multi-disciplinary teams for best practices

• Respond flexibly to youth and family needs

• Assist with tribunal support and EHCP involvement

Requirements:

• Registered social worker with relevant adult care experience

• Knowledge of legislation and obligations for disabled youth

• Full driving license and access to a vehicle

• Strong communication skills in English

• Commitment to prioritizing safeguarding throughout care
