Position: CII Transitional Facilitator for Families
Location: Southwestern Saskatchewan (Maple Creek)

Drive family support as a Transitional Facilitator with CII, focusing on children and youth. Engage in advocacy, case management, and home visits to empower program participants.

CII is seeking a skilled Transitional Facilitator who will use independent judgment to provide comprehensive services to children, youth, and families. This role emphasizes case planning, crisis intervention, and the development of meaningful goals tailored to each family’s needs. You'll collaborate with various community services and conduct essential home visits to assess participant requirements.

Key Responsibilities:

• Conduct regular home visits for needs assessment

• Provide crisis support and intervention

• Develop case plans and community referrals

• Maintain timely documentation and audit-ready records

• Represent CII in community meetings and collaborations

Requirements:

• Master’s Degree in social work or related field

• Preferred case management experience

• Strong skills in child welfare systems

• Valid driver's license with auto insurance

• Bilingual in Spanish/English is preferred
