Serve Stoney Point Healthcare Center as a Full-Time Social Service Director. This role emphasizes emotional support, care planning, and fostering independence among residents.

As a Social Service Director, you will support residents and families in achieving well-being and self-care. This role involves evaluating psychological needs, developing care plans, and coordinating resources with community agencies. You'll document social service status and assist with healthcare decisions and appointments.

Key Responsibilities:

• Assess resident emotional and social needs for tailored care

• Facilitate connections with community resources and services

• Coordinate outpatient appointments and onsite services

• Develop strong relationships with interdisciplinary teams

• Provide counseling to residents and their families

Requirements:

• Bachelor’s degree in Social Work or Human Services

• 2 years of supervised social work experience

• Knowledge of PCC preferred

• Strong communication and listening skills

• Ability to handle emotional and practical challenges
