As a Social Worker with a Bachelor of Social Work (BSW), you'll join the Continuing Care team focused on assessing financial needs and addressing social determinants of health. This full-time role requires ethical decision-making, organization, and collaboration with health care and support professionals.

Key Responsibilities:

• Collaborate with Continuing Care and Home Care teams

• Develop plans to meet financial security needs

• Address social determinants affecting resident wellbeing

• Utilize clinical judgment in problem resolution

• Ensure compliance with ethical standards

Requirements:

• Bachelor of Social Work (BSW) required

• Valid Class 5 driver’s license necessary

• Registered with Saskatchewan Association of Social Workers

• Advanced interpersonal and organizational skills

• Ability to work independently and prioritize tasks
