Job Description
In accordance with the Vision, Purpose, and Values, and strategic direction of the Island Health (Vancouver Island Health Authority), patient & staff safety is a priority and a responsibility shared by everyone at VIHA; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.
Reporting to the Clinical Coordinator, performs professional social work of moderate complexity. Assists patients and their families with practical problems and personal concerns and consults with hospital staff and community agencies regarding the psychosocial aspects of the patients' situation.
Travel between multiple sites may be required.
Social workers work in team-based care and are dealing with clients and their families experiencing trauma, are in crisis, dealing with grief and sudden and unexpected loss, and traumatic change in health condition. Social workers provide short-term supportive counselling, refer clients to various community and government resources and prioritize complex discharge planning. They assist clients in applications for financial, equipment and housing resources.
They initiate conversations with clients and their families around Advance Care planning that includes financial management, health care decision makers, goals of care and end of life wishes. Social workers are involved in complex discharge planning to support clients returning to their home community or an alternate level of care.
All social workers are Designated Responders under the Adult Guardianship Act and must investigate, assess, and intervene in all allegations of adult abuse, neglect and self-neglect. Social workers collaborate with the Ministry of Children and Families on all matters related to child protection. Social workers must be registered with the British Columbia College of Social Workers to practice in hospitals.
Qualifications
Bachelor of Social Work degree (B.S.W.) from an accredited school of Social Work; a Master's degree (M.S.W.) is preferred.
Current full registration with the BC College of Social Workers.
Related social work experience, preferably in a medical setting, and demonstrated clinical competence.
Skills and Abilities
Ability to use and exercise judgment as required.

Knowledge of community resources.
Demonstrated effective oral and written communication skills.
Demonstrated organizational skills.
Ability to establish priorities and organize workload accordingly.
