Service Navigator

Location: Edmonton (West Clareview / East Londonderry)

Overview

The Service Navigator provides coordinated, personalized support to Clients as they move through employment, education, or training pathways. Acting as a consistent point of contact, the role focuses on service navigation, referrals, and coordination, helping Clients understand available services, access appropriate supports, and maintain momentum toward their goals.

Support is delivered using a whole-person, trauma-informed, and strengths-based approach, co-created with the Client to reduce barriers, improve engagement, and ensure continuity across services.

What You'll Do:

Service Navigation & Intake

Serve as the first point of contact following referral; coordinate intake, orientation, and documentation.
Explain program pathways, available services, and expectations in a clear and accessible manner.
Support assessment and planning processes by providing context and building rapport.
Help Clients understand Action Plans and how services align with their goals.

Navigation, Referrals & Coordination

Maintain working knowledge of employment, training, and community-based services and referral pathways.
Work with Clients to identify appropriate services aligned with their goals and Action Plans.
Coordinate referrals and support smooth transitions between services.
Assist Clients in pursuing education or training goals with access to academic, financial, and institutional resources.
Facilitate service coordination meetings as needed to support aligned planning.

Ongoing Engagement & Support

Maintain regular contact to monitor progress, identify barriers, and adjust navigation strategies.
Deliver or coordinate group and individual sessions related to:
Navigating employment and training systems
Job readiness and labour market awareness
Goal setting, confidence-building, and self-advocacy
Support readiness for next steps such as work trials, employment, or training entry.

Documentation & Collaboration

Document navigation activities, referrals, and outcomes in accordance with organizational and funder requirements.
Maintain accurate, timely records to support continuity and reporting.
Participate in interdisciplinary meetings and contribute to service planning.
Uphold trauma-informed, culturally responsive practices.

What You'll Bring:

Required

Diploma or degree in Social Work, Psychology, Disability Studies, Human Services, or a related field.
Minimum 2 years' experience in case management, ideally within employment, wrap around services or mental health.
Proven experience supporting clients with diverse needs and/or barriers.
Knowledge of trauma-informed and strengths-based case management principles.
Demonstrated experience in case management
Demonstrated ability to support individualized goal setting, service navigation, and client progression.
Excellent communication, interpersonal, and documentation skills.
Proficiency in Microsoft Office and comfort using electronic case management or data tracking systems.

Preferred

Experience working in interdisciplinary service delivery models.
Knowledge of inclusive employment principles and the Alberta labour market.
Training in motivational interviewing, conflict de-escalation, or mental health first aid.
Understanding of Indigenous cultural safety and/or lived experience-informed service design.

Job Type: Full-time

Pay: $57,000.00-$66,000.00 per year
