Bayshore Health Care is looking for Social Workers to enrich our Integrated Care Solutions team. You will apply your social work insights to enhance clients' independence and quality of life. With a commitment to family-centered care, your role will involve assessing client needs and collaborating with various stakeholders to facilitate comprehensive psychosocial support.

Key Responsibilities:

• Communicate effectively with clients, families, and agencies

• Efficiently manage and assess a diverse caseload

• Establish and adjust treatment plans with clients

• Enhance clients' coping skills through targeted strategies

• Provide resource recommendations to clients and caregivers

Requirements:

• Active Ontario College of Social Workers membership

• Community-based therapy experience preferred (minimum two years)

• Strong proficiency in English;
French considered an asset

• Must have a current driver’s license and vehicle access

• Clear police background check required
