Case Manager for Colorado Coalition

Location: Quinte Shores, East Northumberland County & Prince Edward County (Picton)

Make a difference as a Case Manager at the Colorado Coalition for the Homeless in Denver. Focus on preventing homelessness while empowering individuals and families to achieve lasting stability.

The Case Manager I is essential in promoting long-term solutions for those affected by homelessness. Collaborating closely with case managers and community partners, you'll develop care plans, coordinate services, and advocate for client needs.

This role offers opportunities in outreach services and crisis interventions, utilizing strengths-based management and trauma-informed care.

Key Responsibilities:

• Manage client caseload focused on chronic homelessness

• Develop and implement individualized care plans

• Facilitate referrals and link clients to services

• Provide crisis intervention and support efforts

• Track client data and outcomes for quality assurance

Requirements:

• 1-2 years experience with diverse populations

• High School Diploma or GED required

• Strong knowledge of trauma-informed care principles

• Valid Colorado driver's license is necessary

• Preferred: bilingual or multilingual skills

Support at-risk populations by fostering care plans and delivering impactful assistance through the Colorado Coalition for the Homeless.
