Your role is to enhance access to health and social services for children living with disabilities or complex medical needs. Collaborate with local communities and organizations to advocate for families and ensure that their needs are met without delays caused by jurisdictional disputes. This position is vital for ensuring families receive timely support.

Key Responsibilities:

• Advocate for children’s service access and resolve jurisdictional disputes

• Ensure coordination of health and social services for families

• Collaborate with BRT6HC and community partners

• Manage case conferencing processes for effective communication

• Promote awareness of the needs of First Nations families

Requirements:

• University diploma in a human service field

• Previous work experience with First Nations

• Understanding of specific health and social issues

• Desire to learn about First Nations cultures

• Case management experience in a related field
