Position: Case Manager for Mental Health Support

In this role, you will report to the Program Manager, offering recovery-oriented case management to individuals with serious mental illnesses. Your responsibilities include direct client engagement, crisis management, and facilitating essential referrals for housing and income support. Throughout your work, you will operate in various community settings while maintaining high professional standards and confidentiality.

Key Responsibilities:

• Manage assessments and treatment plans for clients

• Engage with clients to develop recovery-focused goals

• Provide psychoeducation about symptoms and coping skills

• Liaise with community organizations and healthcare professionals

• Maintain accurate documentation and clinical records

Requirements:

• Bachelor’s degree or diploma in Social Work or related field

• 1-3 years experience in case management

• Knowledge of mental health legislation and services

• Flexibility and strong problem-solving skills
