Cedars is located in Cobble Hill, British Columbia and situated on 60 acres of forested, private land, which provides a picturesque landscape and tranquil therapeutic setting. Vancouver Island is renowned for spectacular West Coast scenery and features a rich network of recovery communities.

Cedars' Continuing Care program provides one year of structured post-treatment support, including one-on-one recovery coaching, facilitated recovery-support groups, alumni programming, family services, and connections to community resources.

POSITION SUMMARY

Reporting to the Manager of Continuing Care, the Recovery Coach supports individuals as they prepare to leave residential treatment and throughout their first year after discharge. Using a strengths-based, patient-centered, non-judgmental coaching approach, the Recovery Coach helps participants identify goals, strengthen recovery capital, overcome practical barriers, connect with community resources, and sustain engagement with recovery supports.

The role combines relationship-based coaching, structured outreach, group facilitation, service coordination, documentation, and team communication. The Recovery Coach works within an approved recovery-coaching scope and promptly escalates clinical, medical, safety, legal, or ethical concerns to the appropriate professional.

DUTIES AND RESPONSIBILITIES

Transition and In-Patient Engagement
  • Build rapport with patients during residential treatment and explain the purpose, boundaries, and available services of Continuing Care.
  • Complete a warm handoff from the primary treatment team and help each participant identify practical, patient-led recovery goals for the transition home.
  • Help participants access approved digital tools, virtual groups, Cedars Trails app, alumni supports, supportive recovery housing information, and relevant community resources.
  • Document contacts and promptly communicate material concerns, barriers, and follow-up needs to the appropriate team member.
Recovery Coaching and Continuing Care Outreach
  • Complete scheduled outreach, coaching contacts, reviews, and milestone acknowledgements throughout the twelve-month Continuing Care period.
  • Use motivational interviewing, active listening, open and evocative questions, goal-setting, and reflective practice to help participants identify their own strengths, choices, and next steps.
  • Support participants to build recovery capital across personal, social, community, cultural, spiritual, housing, health, education, and employment domains, as appropriate to their goals.
