Support individuals to thrive within their communities as a Community Inclusion Worker with Pacific Coast Community Resources. Empower clients to achieve their personal and social goals through collaboration and expert care.
In this role, you will work closely with individuals needing support in personal, social, vocational, and volunteer activities. Your expertise will guide them toward independence and fulfillment while respecting their rights and choices. Collaborate with families and other professionals to ensure all services align with individual goals.
Key Responsibilities:
• Advocate for the rights and choices of supported individuals
• Facilitate community participation in social and vocational activities
• Document daily health information using our case management database
• Build positive relationships with families and team members
• Employ behavior management strategies for effective support
Requirements:
• Grade 12 education and Community Support Worker Diploma
• Minimum 2 years in community social services or healthcare
• First Aid/CPR and Food Safe certificates
• Valid BC Driver’s License and clean Driver’s Abstract
• Criminal record clearance for vulnerable populations
