The Case Manager plays a crucial role in directing the Strengthening Families Program and ensures the development of effective aftercare plans. You will foster collaboration with various departments and support families with tailored assistance, addressing their unique needs. Your work is essential in establishing and maintaining program standards, contributing to family wellness.
Key Responsibilities:
• Manage and guide the Strengthening Families Program delivery
• Create aftercare plans upon case closure
• Ensure comprehensive data management and case practices
• Collaborate effectively with Nation staff and agencies
• Provide clinical oversight for program excellence
Requirements:
• Bachelor of Social Work degree (BSW)
• Two years or more supervisory experience
• Relevant experience with Indigenous children and families
• Strong communication and analytical skills
• Must possess a valid Driver License and undergo a Criminal Record Check
