Nurse Practitioner (NP)with at least 2 years of experience in an outpatient Primary Care setting to join our collaborative and mission-driven team.

Our Canby clinic operates as a Patient-Centered Medical Home (PCMH), offering coordinated care, same-day appointments, on-site labs, and extended hours for patient convenience. • $150,000 base salary + production compensation ? $200,000+/year

• 36 patient hours over a 4-day work week - protect your work-life balance
• 100% outpatient

• NP with at least 2 years experience in an outpatient primary care clinic
• A passion for preventative medicine, with skill in managing both acute and chronic conditions
• Comfort working with patients across the lifespan from young children to older adults
