We are offering an exciting opportunity to join our supportive and friendly Community Nursing team based in Cannock Villages PCN as a Band 5 Registered Nurse. The post is a fixed term contract for 10 months until May 2027 to cover maternity leave, with potential for extension.

The role involves delivering the highest standard of palliative and end‑of‑life care in patients’ homes, with a strong focus on complex, specialist wound and pelvic health care.

Within the community we are seeing more complex and acute patients than ever, requiring highly skilled care. You will support, assess and treat patients with a range of health needs, delivering evidence‑based practice, managing long‑term conditions, supporting acute care needs to avoid avoidable hospital admissions.

You will be required to travel independently and efficiently across a large geographical area. Demonstrate suitability of travel provisions.

• Assess new service user referrals and care/support needs.
• Provide direct care to patients on the caseload with a range of nursing needs, including palliative care, complex wound care, pelvic health and medication administration.
• Promote independence and support patient‑centred care planning.
• Work autonomously within your scope of practice, competence and policies.
• Handover patient information to the wider team and document effectively.
• Escalate patients’ needs as appropriate.

• Degree in nursing or equivalent.
• Mentorship qualification or equivalent.

• Current NMC Registration.
