WeCare is seeking a dynamic and compassionate Hospital Liaison to support our facilities in the Southwest region of Pennsylvania. The ideal candidate has strong interpersonal skills, healthcare knowledge, and a passion for building lasting referral relationships.

• Build and maintain relationships with hospitals, physicians, case managers, and other referral sources.
• Serve as the primary liaison for referrals, providing prompt communication and support to ensure a smooth transition from referral to admission.
• Represent WeCare facilities through marketing outreach and onsite visits to hospitals and other referring agencies.
• Collaborate with Administrators to support both short-term and long-term census development goals.
• Prepare and distribute promotional materials and presentations to referral partners as needed.
• Plan and execute strategic community outreach events.

• Experience in long-term care, admissions, or healthcare sales preferred.
• Strong understanding of local healthcare markets and provider networks.
• Excellent communication, organizational, and relationship-building skills.
• Ability to travel throughout the assigned region.
