In this role, you will support individuals to improve their wellbeing, build connections within their local communities, and develop the skills needed to successfully manage their homes and maintain independent living.

Case load management of 18+ clients with mixed levels of support needs across two supported accommodations.
Liaise with housing agencies to ensure that appropriate referrals are made, including the Local Authority Housing Register and to support clients through homelessness applications with the local authorit...
