• Provide primary and family evidence‑based care, including assessment, diagnosis, treatment, management, counseling, and disposition planning.
• Perform administrative duties such as opening/closing the clinic, inventory, insurance verification, payment collection, and maintaining a clean environment.
• Handle patient appointments and conduct examinations to ensure positive health outcomes.
• Speak with patients about healthy lifestyle choices, complete diagnostic tests, and coordinate with other health professionals for comprehensive care.
• Encourage patients to actively participate in their health and well‑being through screenings, behavioral health, and chronic condition management.

• Minimum one year as a Nurse Practitioner;
  Family medicine or primary care preferred.
• Active Family Nurse Practitioner license in good standing.
• Holistic, evidence‑based care, including pregnancy prevention, STI prevention, and safer sex practices.
• Order and administer immunizations (COVID‑19, influenza, TB, Depo, etc.) and provide vaccine counseling.
• Minimum two years of medically relevant experience or equivalent.
• Complete DOT certification exam within 30 days of employment.
• Proficiency in additional languages (e.g., Spanish) if required by market.
• Basic Life Support (BLS) certification with CPR and AED proficiency.
• Pass FIT test and wear respirator mask when protocol requires.

• Master's Degree level Family Nurse Practitioner program with current National Board Certification and state license to practice as an Advanced Practice Nurse.
• OR, Master of Science in Physician Assistant Studies (or equivalent) with current National Board Certification and state license to practice as a PA.

• EPIC electronic health record experience.

$41.85 – $90.13 per hour (base hourly rate or base annual salary). The actual offer will depend on experience, education, geography, and other factors. The position is eligible for CVS Health bonus, commission, or short‑term incentive programs.
