  In-Home Visits

• Conduct in‑home assessments and functional evaluations
• Deliver personalized, evidence‑based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Work closely with the Director of Clinical Operations to share findings and support process improvement initiatives
• Maintain accurate and timely documentation through our electronic system

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Experience in home health, aging in place, or disability services preferred
• Strong interpersonal and clinical reasoning skills
• Reliable transportation and valid driver's license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

• Contractors paid monthly, within 2 weeks of the previous month’s end
• Pay per completed evaluation includes travel, evaluation, and documentation
• Additional payment for extended travel can be discussed and approved ahead of time
