Signify Health, part of CVS Health, is seeking a Per diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
You'll visit members in their homes for assessments, review medications and histories, perform physical evaluations, and coordinate with health plans to connect members with appropriate care.
