As our RN Case Manager, you’ll orchestrate meaningful care that touches hearts and creates lasting comfort for families during their most vulnerable moments. This role goes beyond medical tasks – you’ll be the steady, compassionate presence that makes all the difference.

  Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences
• Be the Advocate

  Champion each patient’s unique needs, preferences, and wishes while supporting their loved ones

  Build relationships that matter, providing counseling and emotional support when it’s needed most

Your Impact Zone:
As an RN Case Manager you’ll travel throughout the Canton area, bringing healing and hope directly to patients in their homes, assisted living facilities, long‑term care facilities, and hospitals – wherever they feel most comfortable.

• Active RN License
• Valid Driver’s License
• 1+ years hospice experience

• BLS Certification
• Hospice and Palliative Medicine background
