Res Care Community Living seeks a dedicated Licensed Practical Nurse to provide compassionate, client‑centered care in our community living facilities.

Plan and manage patient care based on individualized plans. Conduct assessments, record medical history, administer medications, monitor vital signs, and provide routine support while maintaining client dignity and independence.

• Provide 1:1 care following the client’s plan of care, maintaining dignity and independence.
• Monitor vital statistics, report abnormal findings to case manager and physician.
• Administer medications, feedings, oxygen, and perform ostomy care per client’s plan.
• Complete routines established by allied health professionals.
• Maintain infection control policies and report communicable diseases.
• Accurately document nursing actions, communicate progress to family, case manager, physicians.
• Order supplies and medications as needed to ensure adequate stock.
• Ensure client needs are met per physician’s instructions.
• Monitor incidents, complete incident reports timely, inform case managers.
• Maintain medical equipment, report faulty equipment to Durable Medical Equipment for repair.
• Perform other duties as assigned.

• Completed post‑secondary coursework to facilitate licensure as LPN or VPN.
• Knowledge of individual service plans and client rights.
• Current license as LPN or VPN in good standing from state(s) of practice.
• CPR certified.
• Excellent interpersonal skills with customer service, patience, empathy, compassion.
• Understanding of patient care policies and regulations.
• Familiarity with symptoms and signs of improvement for several medical conditions.
• Moderate travel between facilities may be required.

USD $27.00 - $27.60 per hour.
