Occupational Therapist; Per Diem

As an Occupational Therapist (Per Diem) on our team, you will play a vital role in our mission to provide compassionate healthcare, nursing, and rehabilitation services in an environment of respect, dignity, and personal attention to the needs of the individual.

As an Occupational Therapist (Per Diem) at Terraces of Lake Worth Care Center, you will have the opportunity to work with a diverse range of residents, assessing their physical and cognitive abilities, and developing individualized treatment plans to enhance their overall well‑being. Your expertise will enable our residents to maintain their independence, safely navigate their environment, and participate in activities they enjoy.

• Conduct thorough assessments of residents' physical and cognitive abilities
• Develop and implement individualized treatment plans to address residents' goals and needs
• Provide occupational therapy interventions, including but not limited to, range of motion exercises, strengthening and stretching techniques, and cognition and memory exercises
• Collaborate with other healthcare professionals to ensure comprehensive care
• Maintain accurate records of treatment plans, progress notes, and discharge summaries
• Participate in interdisciplinary team meetings to discuss resident progress and needs
