Remote Patient Care Coordinator

Location: Remote 4pm-12am (South African Time)

Role Overview

The Remote Patient Care Coordinator is responsible for managing and coordinating patient therapy services, ensuring smooth scheduling, excellent patient communication, and efficient support for therapists. This role serves as the primary administrative point of contact between patients, therapists, physicians, and internal staff.

You will help coordinate outpatient physical therapy and occupational therapy services delivered directly in patients homes, ensuring patients receive timely, professional, and compassionate care.

Key Responsibilities Patient Coordination
  • Contact new patients to schedule initial evaluations and therapy sessions
  • Coordinate ongoing therapy appointments between patients and therapists
  • Reschedule or adjust appointments based on patient or therapist availability
  • Maintain accurate patient records in the system
Patient Communication
  • Serve as the main contact for patient inquiries via phone, email, or messaging
  • Provide appointment reminders and follow-ups
  • Ensure patients understand their therapy schedule and next steps
  • Deliver excellent customer service to elderly and post-surgical patients
Administrative Support
  • Capture and update patient demographics, insurance details, and documentation
  • Support intake process for new patients
  • Track patient progress and therapy frequency
  • Assist with reporting and administrative tasks
Therapist Coordination
  • Work closely with physical and occupational therapists
  • Coordinate therapist schedules and caseloads
  • Ensure efficient scheduling routes and time management
  • Communicate schedule changes promptly
Insurance & Intake Support (Optional / Bonus)
  • Verify insurance eligibility (Medicare or private insurance)
  • Assist with patient onboarding and intake forms
Requirements Essential
  • Experience as a Patient Coordinator, Medical Administrator, or Customer Support Agent
  • Excellent English communication (spoken and written)
  • Experience coordinating appointments or schedules
  • Strong organizational and multitasking skills
  • Experience working remotely with US clients preferred
  • Stable internet connection and quiet work environment
Preferred
  • Healthcare coordination or medical administration experience
  • Experience working with elderly patients
  • Experience with EMR/EHR systems
  • Experience in Physical Therapy, Home Health, or Rehab environments
