We are looking for a Care Navigator to join the team at Debenham Group Practice. This is a patient-facing role at the heart of how the practice runs day to day, and we are looking for someone who will bring a calm, friendly and organised approach to supporting our patients and colleagues.

We care for 10,000 patients across our main site in Debenham, with branch surgeries at Otley and Grundisburgh.

We are looking for someone who:
  • Is calm, warm and professional when dealing with patients and colleagues
  • Listens carefully and records information accurately
  • Is organised, reliable and takes their responsibilities seriously
  • Works well as part of a team and is willing to support colleagues
  • Has good IT skills and picks up new systems with confidence
  • Has previous experience in a customer-facing or administrative role

Experience in a healthcare or NHS setting is helpful but not essential we are happy to support the right person to develop in the role.

Main duties of the job

Care Navigators are often the first voice or face a patient encounters. Whether patients contact us by phone, walk through the door, or submit a request online, you will help make sure they get the right information and reach the right place.

We use askmyGP as our primary workflow tool for clinical requests. Rather than triaging patients themselves, Care Navigators help ensure that requests are accurately recorded so that the duty GP can review and act on them promptly. You will also take turns supporting the duty GP as part of the wider Care Navigation team, and you will help signpost patients to other local services where that is the most appropriate response.

The role also covers the full range of day-to-day practice administration from reception duties and managing records, to supporting clinical and dispensary colleagues and keeping the practice running smoothly.

You will work across our sites as needed, as part of a well-established, friendly team.

We are looking to recruit for Wednesday, Thursday and Friday. Shifts fall within a working window of approximately 07:30 to 18:30 across those days. Depending on recruitment, hours may be split across more than one person.
