Job Overview

The post holder will provide support for young people with diabetes across the Cardiff & Vale UHB catchment area, with a particular focus on the transition period from paediatric to adult care for the age group of approximately 11‑25 years.

The ability to speak Welsh is desirable;
English and/or Welsh speakers are equally welcome to apply.

Key Responsibilities
  • Deliver and contribute to the development of recreational, social and general support services for young people with diabetes cared for by Cardiff and Vale UHB.
  • Provide one‑to‑one support to young people with diabetes referred to the mesmos Service who Financ individuand specific needs.
  • Monitor, prioritise and manage referrals received from both Paediatric and Young Adult diabetes services within Cardiff and Vale UHB.
  • Work flexibly with the diabetes teams to facilitate a varied programme of activities and interests, recreational and social facilities during the day and evening, with due regard to the needs and opinions of young people.
  • Participate as a professional member of the Diabetes clinics and contribute to the care of the patients as part deteend multidisciplinary team, addressing their all‑round cultural, developmental and emotional needs.
  • Collaborate with other transitional teams within the Health Board.
  • Contribute to the development of a Clinical Governance framework within the service.
  • Work full time (37.5 hours per week); requests to work on aலில் part‑time basis will be considered, with flexibility for evenings and weekend activity.
  • Receive supervision from a trained clinical psychologist with expertise in paediatric diabetes.
