VAP Senior Assessor

The Virtually Assessed Pathway clinic (VAP) is an award winning service, unique to Velindre Cancer Centre, This post holder will be joining a small friendly team of nurses and pharmacy technicians led by independent prescribers. The main aspect of the role will be to work as part of a team within the telephone assessment clinic to clinically assess a patients fitness to proceed with the next cycle of SACT (Systemic Anti Cancer Treatment) using a clinically robust assessment form, under the supervision of an independent prescriber.

There may be a requirement to work occasionally in the face to face pre SACT assessment clinics.

The VAP service is not just a pre SACT assessment clinic however, we manage the patients  cancer pathway and as a result we become part of their journey. We maintain continuity of care and as a result we develop a relationship with our patients as we often assess them before each SACT for months at a time.

The VAP clinic currently works with the following teams assessing patients on suitable regimens from the following cancer sites

Colorectal / Breast / Urology / Gynae / Skin/ UGI / Hepatobiliary/ Lung .

The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care, participating and contributing to the wider clinical governance agenda including service developments and flexible working to meet the needs of patients.

The working week days are flexible. Experience of working in oncology is essential and the SACT administration passport is desirable for this role.

• Current NMC or HCP registration
• Professional knowledge acquired through degree supplemented by post graduate specialist training, experience, short courses plus further specialist training to master s equivalent level. oClinical assessment and formulation skills
• Knowledge of clinical governance processes and how they underpin care delivery
• Knowledge of research/audit processes and application in practice
• Management of patients with complex needs
• Oncology/Haematology

• Ability to meet the requirements of the role
• Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading and writing in Welsh
