Clinical Pharmacist
We are seeking a motivated and patient focused Prescribing Pharmacist to join our clinical team. The successful candidate will play a keyrole in delivering high quality pharmaceutical care, supporting safe and effective prescribing, and improving patient outcomes across a range of clinical services.
The Prescribing Pharmacist will work collaboratively withGPs, nurses, and multidisciplinary healthcare professionals to assess patients,manage long term conditions, conduct medication reviews, and prescribe within their scope of practice. This role requires strong clinical knowledge,excellent communication skills, and a commitment to delivering patient centred care.
We are looking for 25-30 hours depending on experience.
Main duties of the jobKey Responsibilities
- Improve the quality and effectiveness of medicines management in the practice
- Prescribe medications safely and appropriately as an Independent Prescriber
- Perform structured medication reviews and medicines optimisation
- Provide patient focussed healthcare
- Support management of chronic diseases such as hypertension, diabetes, asthma, and COPD
- Provide advice and education to patients regarding medicines and treatment plans
- Maintain accurate clinical records and ensure compliance with professional standards
- Contribute to service development and quality improvement initiatives

Job responsibilitiesWe are looking for an experienced and patient centred Prescribing Pharmacist to join our healthcare team. The successful candidate will be responsible for delivering high-quality clinical pharmacy services, supporting safe and effective prescribing, and improving patient outcomes through medicines optimisation and evidence based care.
The Prescribing Pharmacist will work collaboratively with GPs, nurses, and other healthcare professionals to assess, treat, and manage patients within their scope of practice while ensuring compliance with all professional and regulatory standards.
Key Responsibilities
- Ensure compliance with NHS guidelines, local policies, and professional standards
- Develop and embed safe, efficient prescribing processes within the practice
- Contribute to clinical audits, quality improvement initiatives, and service development
- Support safer prescribing of high risk medications e.g. anticoagulants, opioids, DMARDs
- Conduct medication reviews
- Prescribe medications safely and appropriately as an Independent Prescriber
- Support the management of long-term conditions including hypertension, diabetes, asthma, COPD, and cardiovascular disease
- Review and optimise patient medications to improve safety, adherence, and clinical outcomes
- Design, implement, and refine protocols, SOPs, and medicines workflows
- Identify and reduce prescribing risks, polypharmacy issues, and medication errors
- Provide patients with advice and education regarding medicines and treatment plans
- Maintain accurate and up to date clinical records
- Participate in continuing professional development and mandatory training
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
- Master's degree in Pharmacy (MPharm) or equivalent recognised pharmacy qualification.
- Post-registration experience as a pharmacist in primary care, general practice, community pharmacy, hospital pharmacy, or a related clinical setting.
- Experience undertaking medication reviews and medicines optimisation.
- Knowledge of prescribing guidelines, clinical governance, and medicines management.
- Experience working as part of a multidisciplinary healthcare team.
- Excellent communication and patient consultation skills.
- Experience using electronic prescribing and clinical record systems.
