About the Role

As a Social Worker, you will support adults experiencing mental health challenges within the community, helping them achieve greater independence, recovery, and wellbeing. Working as part of a multidisciplinary team, you will assess needs, develop personalised care plans, coordinate services, and safeguard vulnerable individuals.

Key Responsibilities

Your duties will include:

  • Managing a caseload of adults with mental health and social care needs.
  • Conducting comprehensive social care assessments and reviews in line with the Care Act 2014.
  • Developing and implementing person-centred care and recovery plans.
  • Supporting service users and carers through a recovery-focused approach.
  • Working closely with healthcare professionals, local authorities, and community partners.
  • Leading adult safeguarding investigations and promoting best safeguarding practices.
  • Carrying out risk assessments to protect vulnerable adults and service users.
  • Commissioning appropriate health and social care services where required.
  • Maintaining accurate electronic care records and documentation.
  • Providing guidance, supervision, and mentoring to newly qualified social workers and unregistered social care staff.
  • Participating in continuous professional development to maintain Social Work England registration.
  • Undertaking or working towards Approved Mental Health Professional (AMHP) training where applicable.
Requirements

Applicants should have:

  • Registration with Social Work England
    .
  • Experience working in community mental health or adult social care.
  • Strong knowledge of the Care Act 2014 and safeguarding legislation.
  • Excellent assessment, communication, and care planning skills.
  • Experience working within multidisciplinary healthcare teams.
  • Commitment to recovery-focused and person-centred care.
  • A passion for supporting people with mental health needs.
