NHS Orkney is seeking an enthusiastic Primary Care Clinical Pharmacist (Polypharmacy & Medicines Reablement) to join our Primary Care Pharmacy Team at The Balfour, Orkney.
You will conduct structured polypharmacy reviews, medicines reconciliation and optimisation activities across GP practices, care homes, community settings and patients' homes, aiming to reduce medicines-related harm and promote independence.
