Hours: Full-time – 40 hours a week, 12-hour shifts + 4 admin hours

About the Role

Believe Care and Support is looking for an experienced, confident and compassionate Team Leader to join a small, specialist team supporting a young woman in her own home in Lytham St Annes.

This is a highly rewarding opportunity for someone with strong experience in mental health, trauma-informed care, eating disorders and complex support needs, who is ready to take on a leadership role while remaining actively involved in delivering high-quality, person-centred support.

The individual we support has previously experienced a long-term hospital admission and currently receives 3:1 support. She lives with Dissociative Identity Disorder (DID), an eating disorder and autism, and requires a calm, consistent and trauma-informed approach.

As Team Leader, you will play a key role in providing stability, consistency and positive leadership within the team. You'll work closely with the Service Support Manager and management team, healthcare professionals, advocates and the wider support network to ensure support is safe, effective, consistent and centred around the individual's choices and goals.

This is a role for someone who can lead by example, build trusting professional relationships and remain calm and emotionally regulated when situations become challenging.

Key Responsibilities

As Team Leader, you will:

• Lead and support a small team of Support Workers, providing guidance, direction and positive role modelling.
• Ensure consistently high standards of person-centred care and support.
• Provide hands-on support as part of the 3:1 team.
• Promote a trauma-informed, psychologically informed and compassionate approach to support.
• Support the individual with emotional regulation, routines, independence and daily living skills.
• Understand and appropriately respond to complex mental health presentations and behaviours that may arise from trauma.
• Support with eating-disorder-related needs in accordance with support plans, risk assessments and professional guidance.
• Promote positive risk-taking, independence and meaningful choice.
• Support community access, appointments, social activities and hobbies.
• Support medication management and ensure records are completed accurately.
• Ensure support plans, risk assessments and daily records are followed and maintained appropriately.
• Communicate effectively with healthcare professionals, family members and the wider multidisciplinary team.
• Help identify changes in presentation, wellbeing or risk and escalation concerns appropriately.
• Support new team members with induction, training and understanding the individual's needs.
• Assist with team communication, handovers and maintaining consistency across shifts.
• Contribute to reviews and ongoing development of the support package.
• Maintain professional boundaries while developing positive, trusting relationships.
• Promote dignity, privacy, choice and independence at all times.
• Assistance with staffing and rota scheduling.
