An exciting opportunity has arisen for a highly motivated, enthusiastic and passionate clinical pharmacist with a keen interest in renal services to join our expanding integrated pharmacy team. You will become part of a unique team in a highly patient focused role. You will be responsible for leading and developing clinical pharmacy services to patients with renal disease across the Central Integrated Health Community (IHC).
The Central IHC manages patients with chronic kidney disease, including those who require advanced treatment options, such as dialysis. The successful candidate will provide support to the multidisciplinary team for these patients whilst inputting into transformative changes in how care is being delivered across the IHC.
The successful candidate will be a specialist clinical pharmacist, with or able to work towards an independent prescribing qualification. You will be able to work well as part of a ward-based team and across the wider Central IHC, providing leadership and clinical expertise to other pharmacists and technicians within the IHC
Main duties of the jobThe successful candidate will be supported to develop the scope of the role, supporting the renal unit, Emergency Quarter and wards to review patients with acute and chronic kidney disease who are admitted to hospital and in community settings. The renal unit at Glan Clwyd treats patients with varying degrees of renal function and those requiring acute or chronic dialysis treatments.
