** CHC Nurse Practitioner required in Cardiff for 3-month contract Excellent hourly rates*
* We are working with an esteemed client based in Cardiff and urgently require a Nurse Clinical Practitioner for Case reviews of patients with Continuing Healthcare needs (CHC)
This will be a 4 Month project covering 120 case reviews and is full time Monday - Friday, the role will be site based predominantly but will include some travel to patients homes.

The Practitioner would have responsibility for:

• initial reviews

• arranging and facilitating DST reviews and recommendations

• All appropriate recording and documenting outcomes

To be considered for this role following criteria is essential:

• NMC Registered Nurse

• Current experience working in the same or very similar role
