Telephone Triage Practitioner

Job Overview

The post holder will undertake telephone triage assessments of patients, using best clinical practice and adhering to Cardiff and Vale policies, procedures and guidelines.

Clinical telephone triage of patients contacting Urgent Care Services within Cardiff and Vale LHB, managed by the CAV 24/7 business unit. Clinicians will be supported within this role by using accredited decision support templates.

• Conduct telephone triage assessments efficiently and effectively.
• Implement the most appropriate intervention based on patient needs.
• Use decision support templates to guide assessments.
• Provide triage outcomes including self‑care advice, face‑to‑face appointment referral, redirect to EU, or referral to other services within Cardiff and Vale localities.
• Support the multidisciplinary team (MDT) approach to patient care within the community setting.
