Senior Nurse

The Senior Nurse will proactively identify patients approaching the end of their lives, ensuring supportive conversations are held to enable patients and their loved ones to discuss preferences and wishes. Working with Health Boards' teams and patients’ families, they will carry out holistic assessments, support fast‑track assessments, and develop ongoing care and treatment plans, including advance care planning and comprehensive treatment escalation plans.

The Senior Nurse will be part of the Health Board’s discharge team, collaborating with multidisciplinary and specialist palliative care teams to ensure patients are safely and timely discharged to an alternative care setting that meets their preferences and needs. They will also closely work with Marie Curie’s Hospice Care at Home Service to utilise capacity and facilitate community discharges.

Salary
: £40,559 – £48,841 pro rata per annum (aligned with the Marie Curie Clinical Pay Scale Band 6) plus enhancements.

Contract
:
Permanent – Part‑Time, 22.5 hours per week (09:00‑17:00 Monday‑Friday).

Based
:
Cardiff and the Vale Hospice, plus community locations.

• Registered Nurse on the NMC Register with an appropriate Nursing degree.
• Proven experience in palliative care (hospice, hospital or community).
• Strong communication and teamwork skills.
• Ability to manage workload and prioritise tasks effectively.
• Commitment to continual professional development and reflective practice.
