Velindre Cancer Centre – Clinical Nurse Specialist –Breast Band 7

We have an exciting opportunity for an experienced nurse to join our clinical nurse specialist team at Velindre Cancer Centre. You will have experience in breast cancer care, oncology or palliative care and be passionate about delivering high quality care to this group of patients.

As part of the breast site specific team you will be responsible for delivering a quality support service to patients with breast cancer.

You will join a supportive, forward thinking and innovative multidisciplinary team who are committed to improving services and supporting patients on their cancer journey

You must have an understanding of the needs of cancer patients; possess excellent communication skills for advising and supporting patients, relatives and other health professionals along with knowledge of managing a range of conditions, including disease progression complications, and management of side effects of systemic anti-cancer treatment and radiotherapy.

You will be dynamic with an ability to work autonomously, prioritise care and balance workload in a busy environment whilst supporting colleagues. You will work in the outpatient setting and ensure that appropriate pathways are in place to follow the patient throughout their treatment, clinic and inpatient settings.

Main duties of the job
  • Act as a patient’s key worker and support the coordination of patient care along the treatment pathway
  • Ensure patients are offered holistic needs assessments at relevant points throughout the pathway
  • Assess patient information needs and meet those needs as required
  • Clinically review and assess cancer patients in the Outpatients setting

This post is fixed term/secondment for 12 months. Detailed job description and main responsibilities
  • Attend outpatients clinics in order to meet patients and their carers and provide relevant information to support their treatment
  • Provide psychological support and make referrals to relevant professionals when appropriate
  • Act as a key worker for a clinical caseload of patients, working to coordinate care and provide easy access for ongoing support for patients
  • Provide effective communication between patient, relative/carer and MDT
