Practice Nurse - North Medical Centre

The Practice Nurse will provide:

• assessment, treatment, screening and health promotion services and advice working in accordance with the NMC Code of Conduct and within their scope of practice.
• nursing services to patients independently or in participation with Nurse/GP colleagues working to agreed protocols and guidelines., Provide clinical services as designated by the Nurse Manager/Clinical Governance Lead and Provision of practice Nursing duties
• Work with other health care professionals to monitor, manage and treat long-term conditions, in line with national and local policies and practice needs.
• Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols.
• Work with patients in order to support adherence to prescribed treatments
• Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care
• Support and manage health needs of women presenting for family planning and cervical cytology consultations.
• Participate in vaccination and immunization programmes for both adults and children.
• Undertake immunisations, injections and vaccinations
• Undertake screening, e.g. blood pressure, cervical cytology.
• Management of medical/nursing supplies, including stock control and expiry checks.
• Maintenance of treatment room and clinical equipment including testing and recording where so defined and adherence to infection control procedures.
• Maintain accurate comprehensive records of all consultations and treatments in the patient record and completion of all necessary documentation associated with patient health care.
