What you will do:

  • Conduct comprehensive assessments of individuals with learning difficulties to identify needs and develop tailored care plans.
  • Collaborate with service users, families, and carers to provide holistic support, ensuring all perspectives are considered in the care process.
  • Liaise with multi-disciplinary teams, including health professionals and external agencies, to coordinate effective and seamless service delivery.
