Job Summary
An exciting opportunity for a permanent band 7 Senior Urgent Care Practitioner to join the community team supporting the Ceredigion Same Day Urgent Care/Outreach service. The post requires an enthusiastic, motivated, and dynamic practitioner with broad understanding and experience of acute and chronic conditions. Experience of autonomous practice in the emergency/urgent/MIU arena is essential.
The team aims to ensure the best outcomes for patients, supporting continuing professional development and improvement in integration across community with same day emergency care provision.
Main duties- Conduct advanced assessments and develop care plans with community/primary and acute MDT.
- Triage and assess patients, carry out treatments, work closely with ACPs and community MDT.
- Deliver diagnostic skills, multi‑tasking and manage priorities effectively.
- Commit to service improvements, adapt to change and work in a positive environment.
- Shift work: 7 days a week, day shifts 08:00‑20:00 and night shifts 19:30‑08:30.

Person Specification Essential- Registered Practitioner (NMC/HPC)
- CPD to Masters level
- Level 7 module covering advanced assessment, clinical examination and diagnosis skills for minor injury and illness
- ALS / EPALS and in‑house Trauma Training or ATLS
- Evidence of CPD and awareness of Clinical Governance
- A&E Nursing / Urgent Care / Community or Primary Care experience
- Ability to relate theory to practice and maintain personal and organisational development
- Provide specialist clinical advice within multidisciplinary team
- Administer drugs in accordance with local PGDs
- RSCN
- Teaching qualification
- Management qualification or equivalent
- Awareness of topical issues in health and social care
- Non‑Medical Prescribing
- Welsh speaker (Level
1)
- Relevant experience as a registered nurse in minor injuries/illness/urgent/community or acute care
- Experience in a senior capacity within an MDT
