Working within a Mental Health team, responsible for assessing social care needs and delivering person-centred support to individuals and groups across Cumbria. Managing a caseload to coordinate outcomes that promote independence, choice, and wellbeing, while effectively managing risk and working within financial guidelines. Using a strengths-based and reablement approach, the role involves linking people to community resources, preventative support, and statutory services.
