About the Role
We are looking for an Older Adults Mental Health Service Consultant to join a caring and committed team delivering high quality services to our later life population in West Wales. As an Old Age Consultant Psychiatrist you will work across the pathway of primary, community and specialist secondary care, ensuring continuity of care for older adults with mental ill health.
Key Responsibilities- Assess patients at the initial referral stage, whether in the patient’s home, care home, or in clinic.
- Lead multidisciplinary team (MDT) formulation of risk and treatment plans.
- Provide ongoing management of episodes of later life mental ill health.
- Clinically lead admission to the older adults mental health ward when appropriate.
- Coordinate care with General Adult services, GH, and local mental health liaison services.
- Maintain continuity of care for patients admitted to General Hospital, ensuring seamless transition.
- Knowledge and experience of the Mental Health Act, Mental Capacity Act and the Mental Health (Wales) Measure.
- Section 12 MHA and Approved Clinician status.
- Experience managing functional and organic disorders in later life.
- Ability to work collaboratively across service and specialty boundaries.
- Strong commitment to continual professional development and research.
