An exciting opportunity has become available for a Band 5 Well-being Practitioner to work within our Carmarthenshire Primary Mental Health Service.
Individuals will be expected to be enthusiastic and committed to provide well-being and mental health support to help address locally identified need and sustain capacity within wider Primary Care Mental Health Services. Well-being Practitioners will deliver timely interventions in the community, which will ensure early intervention, preventing many individuals from requiring secondary mental health services.
The ability to travel between sites within the county in a timely manner is essential.
Main duties of the jobThe post holder will use a range of communication methods adaptable to the needs of individuals, families/carers and partner organisations to support individuals living with symptoms of mild mental illness.
The post holder will provide support to the Service at large, to enable the best possible care outcomes for those experiencing mental health distress and connect individuals with local Mental Health and well-being services where appropriate, including liaising with partner organisations such as GP's, Primary Care, WAST, A&E, Police and Third Sector etc.
Provide general mental health advice, relating to clinical risk, Mental Health Act or Mental Capacity Act.
Prioritise own workload to ensure that all individuals receive a timely intervention.
Develop and maintain relationships with Health Board services and partner agencies such as GP's, 111 Option 2, Police, Third Sector, Local Authority and WAST etc.
The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.
Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

- Four main hospitals:
Bronglais Hospital in Aberystwyth;
Glangwili Hospital in Carmarthen;
Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest - Five community hospitals:
Amman Valley and Llandovery hospitals in Carmarthenshire;
Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire - Two integrated care centres:
Aberaeron and Cardigan in Ceredigion, and several other community settings - 47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Person Specification Qualifications and Knowledge- Educated to graduate/ diploma level in a related field e.g psychology, social care, occupational therapy OR equivalent demonstratable experience of working at graduate/ degree level
