Position: Integrated Care Clinician (Nurse) - Llangunnor

Integrated Care Clinician (Nurse) – Llangunnor

The post holder will undertake telephone triage assessments of patients efficiently, advising the most appropriate intervention using best clinical practice and adhering to Trust policy, protocols and guidelines. Work will be undertaken autonomously, assessing and advising callers within agreed guidelines.

Main duties of the job

As a Band 6 Triage and Assessment Nurse you will support urgent and emergency care across Wales, drawing upon your clinical assessment skills to advise a broad spectrum of patients, from minor ailments to complex urgent conditions. You will make clinical decisions remotely alongside a multidisciplinary team of senior clinicians, practice coaches, GPs, pharmacists and mental health practitioners.

Person Specification Qualifications and/or Knowledge & Experience
  • NMC Registered Nurse (adult or child) with relevant clinical practice in front‑line, acute or chronic care, or telephone triage; or HCPC Registered Paramedic with post‑registration experience in front‑line services.
  • Evidence of undertaking regular update training and continuous professional development as required by the regulating body.
