Integrated Care Clinician (Nurse)
The post holder will undertake telephone triage assessments of patients efficiently, advising the most appropriate intervention using best clinical practice and adhering to Trust policy, protocols, and guidelines.
They will work autonomously within agreed guidelines, assessing and advising callers in an efficient manner.
Main duties of the jobAs a Band 6 Triage and Assessment Nurse you will use clinical assessment skills to assist and advise a broad spectrum of patients, from minor ailments to complex urgent conditions, in a remote clinical decision‑making environment.
You will have the autonomy to make decisions while being supported by experienced senior clinicians, practice coaches, and multi‑disciplinary colleagues.
You will work closely with urgent care GPs and pharmacists, engaging thousands of patients each week.

Detailed job description and main responsibilitiesThe ability to speak Welsh is desirable but not essential. English speakers are also welcomed.
Person specificationQualifications and/or Knowledge & Experience
- NMC Registered Nurse (adult or child) with relevant clinical practice on the front line, acute or chronic care, or telephone based triage; or HCPC Registered Paramedic with post‑registration experience on front line services.
- Evidence of regular update training and continuous personal/professional development as required by the regulating body.
- Able to demonstrate strong listening and communication skills.
- Team working with a collaborative style.
- Work independently and make decisions on own initiative.
- Sound clinical decision‑making capabilities.
- High clinical knowledge, computer literacy, commitment to ongoing development.
- Committed to mandatory training updates.
- Ability to work under own initiative without direct supervision.
- Ability to speak Welsh.
- Able to demonstrate the Trust values and behaviours.
- Flexible approach to work – expect rotating shift system including nights and weekends (unsocial hours).
